Makeup Practice Request
This form is for gymnasts in the Team Program only. If your gymnast takes classes (including pre-team), there is a separate form in the parent portal for you to submit makeup requests.

Please request all makeups at least 7 days in advance of your desired makeup date. If your submission is sent within 7 days of your requested makeup, we may not be able to accommodate your request. Makeups are issued on a first come, first served basis, and are based on availability. Priority is given to makeups due to any class cancellations. A confirmation email will finalize your requested makeup.
